With 57 years of combined nursing experience and more than 20 years of I/DD experience, we understand the challenges, questions, and important decisions families face as their loved ones transition into adulthood.

We specialize in helping individuals and their families navigate the transition from the Children's Extensive Supports (CES) Waiver to the adult Developmental Disabilities (DD) Waiver. Working closely with case managers and support teams, we help families understand their options, maintain essential home and community-based services, and create a plan centered on each individual's goals, strengths, and future.
